Public Notice: PARTNERS IN COMMUNITY NURSING PRIVACY INFORMATION PRACTICES

Personal health information is information in any form that identifies you and relates to your health and health care including health history, health care programs and services, health care providers, substitute decision maker, health care number and other personal identification numbers.

  • All personal health information is secured and kept private.

  • Unless the law requires disclosure which authorizes sharing of health information with others, Partners In Community Nursing cannot release any personal health information without the written consent of the individual involved.

  • Individuals may give written consent for family or legal representatives to access their personal health information. These representatives may have access to only those parts of the personal record that have been identified in the consent.

  • Access by an individual to request a correction to their health information must be in writing and delivered to Partners In Community Nursing.

  • Partners In Community Nursing is aware of the sensitive nature of your personal health information and have been trained in the appropriate use, disclosure and protection of your personal health information.

  • Effective October 1st, 2016, processing of personal health information will be outside of Ontario, using a facility located in Quebec, Canada.

Individuals have the right to withdraw or change the conditions of the consent, subject to provisions set out in the Personal Health Information Protection Act, 2004 (PHIPA).

Under the Personal Health Information Act, 2004, Partners In Community Nursing and its Quality Coordinator are considered information custodians.
